When paying for service online choose the 1040 form and then go back and choose the state(s) form, be sure to include the same number of states you have W-2's for in the quantity field. Any other forms will be listed at the end of the questionaire. You will be asked to download the questionaire at the end of your order process. You will be billed for any extra forms you may need at the time of preparation
